P25-10 High sensitive troponin-I can be an effective predictive marker for drug-induced myocardial injury

Progression of chemotherapy may prolong overall survival in advanced cancer, and prediction drug-induced myocardial injury is important for continuing treatment. In particular, anthracycline drugs can cause irreversible cardiac dysfunction. Thus, predictive biomarkers cardiotoxicity are needed. We retrospectively reviewed patients who were treated with our hospital between January 2017 June 2020. 323 analyzed (203: breast 80: malignant lymphoma, 21: gynecological malignancy, 19: soft tissue tumor). Drug-induced was observed 21 (13: 6: 2: tumor) (6.5%). One case fatal. Pre-treatment echocardiographic screening performed 246 cases (76%); on the other hand, post-treatment echocardiography only 103 (32%). Brain natriuretic peptide (BNP) measured 15 (4.6%). A 38-year-old male tumor 1st-line pazopanib, 2nd-line eribulin, 3rd-line doxorubicin. checked BNP toroponi-I (Tn-I) during pre-treatment. After 5 cycles doxorubicin, Tn-I showed a positive value; at this time, stayed within normal limit. then stopped doxorubicin treatment monitored patient. not detected healthy people, has high specificity sensitivity. The algorithm to monitor chemotherapy, Tn-I+-BNP, recommended. Like case, these detect before ejection fraction decreases. be an effective marker injury, especially risk populations such as seniors, females, trastuzumab use after drugs.
